Tirzepatide is administered via a once-weekly injection using the KwikPen, given subcutaneously into the stomach, thigh, or upper arm — on the same day each week.
Rotate your injection site weekly. Even within the same general area, choose a slightly different spot each time to reduce the risk of lumps forming under the skin.
Treatment follows a structured titration schedule rather than starting at full strength. According to MIMS Malaysia, the starting dose is 2.5 mg once weekly, increasing to 5 mg after 4 weeks, with further increases in 2.5 mg increments after at least 4 weeks on the current dose.
Storage matters: keep pens refrigerated at 2°C to 8°C before use. Never freeze, and never leave a pen in a car — Malaysian interior temperatures will degrade the medication within minutes.

Once-weekly Tirzepatide Or Mounjaro has multiple dose options
Mounjaro comes in six KwikPen strengths: 2.5 mg, 5 mg, 7.5 mg, 10 mg, 12.5 mg, and 15 mg.
Treatment begins at 2.5 mg once weekly for the first 4 weeks, then increases to 5 mg weekly depending on your response and your doctor’s guidance.
The 2.5 mg starting dose is deliberately sub-therapeutic. It exists purely to let your digestive system adjust. Escalating too quickly is the main cause of severe nausea, and the most common reason people abandon treatment entirely.
What happens next?
At your check-up, tell your doctor how you’re responding to 2.5 mg and 5 mg. After reaching 5 mg, you may remain at that dose longer than 4 weeks — there’s no requirement to keep climbing if you’re doing well.
Your doctor may recommend increasing to 7.5 mg, 10 mg, 12.5 mg, or 15 mg. Each dose level is maintained for at least 4 weeks before any further increase.

Mounjaro Malaysia: Availability, Indications, and Approval Status
Mounjaro (tirzepatide) became officially available in Malaysia on 30 August 2025, following approval by the National Pharmaceutical Regulatory Agency (NPRA). It is a prescription-only medication indicated primarily for type 2 diabetes management.
Legal Status Under the Poisons Act 1952
Tirzepatide, like all GLP-1 receptor agonists in Malaysia, is classified as a Group B Poison under the Poisons Act 1952.
It may legally be supplied only by a registered medical practitioner treating their own patient, or by a registered pharmacist against a valid prescription.
Supplying it outside these conditions is a criminal offence. Importing personally from overseas without an NPRA permit risks customs seizure under the Sale of Drugs Act 1952.
