Quick answer: Wegovy (semaglutide 2.4 mg) is approved as an add-on to a reduced-calorie diet and more physical activity for adults with obesity, or with overweight who also have at least one weight-related health condition, such as high blood pressure, type 2 diabetes or high cholesterol. It is not suitable for everyone. Avoid it if you or a close family member has had medullary thyroid carcinoma, if you have MEN 2, if you have had a serious allergic reaction to semaglutide, or if you are pregnant or planning pregnancy. A doctor must assess your full health history first.

Wegovy is one of the most widely discussed weight-management medicines, and many people ask whether they qualify. Eligibility depends on your weight, health conditions, medical history and current medicines. This guide explains the approved criteria, who should avoid Wegovy, who needs extra caution and what to bring to your consultation. For the treatment overview, see our Wegovy treatment guide.

What Wegovy Is Approved For

Wegovy is a prescription medicine for chronic weight management. In the US prescribing information it is approved for adults with obesity (BMI 30 kg/m² or higher), and for adults with overweight (BMI 27 kg/m² or higher) who have at least one weight-related condition such as hypertension, type 2 diabetes or dyslipidaemia. It is also approved for adolescents aged 12 and older with obesity, and for reducing the risk of major cardiovascular events in certain adults with established heart disease and obesity or overweight. The approved use can differ by country, so confirm the Malaysian product information with your doctor.

Who May Be Suitable

Your doctor may consider Wegovy if you:

  • Are an adult with obesity, or with overweight and at least one weight-related health condition
  • Have tried diet and exercise changes, and need additional support
  • Have no contraindications, and can manage a weekly injection or the approved form
  • Are willing to attend follow-up appointments to monitor weight, side effects and blood tests
  • Have realistic expectations about the amount of weight loss and the need for long-term use

Being overweight alone is not enough. Your doctor will weigh the benefits and risks for your own health.

Who Should Avoid Wegovy

Wegovy must not be used in these situations:

  • Personal or family history of medullary thyroid carcinoma (MTC). Wegovy carries a boxed warning about thyroid C-cell tumours.
  • Multiple Endocrine Neoplasia syndrome type 2 (MEN 2). This inherited condition increases the risk of MTC.
  • Serious allergic reaction to semaglutide or any ingredient. Anaphylaxis and angioedema have been reported.
  • Pregnancy or planned pregnancy. Wegovy may cause fetal harm. Stop at least 2 months before you plan to conceive, because semaglutide stays in the body for a long time.

If MTC or MEN 2 runs in your family, tell your doctor even if you are unsure of the details.

Who Needs Extra Caution

Some people can use Wegovy only after careful review. Tell your doctor if you have:

  • A history of pancreatitis or gallbladder disease. Both have been reported with semaglutide.
  • Diabetic eye disease. Rapid blood sugar improvement can worsen existing retinopathy in people with diabetes.
  • Kidney problems. Vomiting or diarrhoea can cause dehydration and kidney injury.
  • Depression, anxiety, or a history of suicidal thoughts. The prescribing information advises monitoring mood, and avoiding use in people with active suicidal ideation or a history of suicide attempts.
  • Diabetes medicines. Insulin or sulfonylureas can increase the risk of low blood sugar.
  • An eating disorder or a history of disordered eating. Weight-loss medicines need careful assessment in this situation.
  • Plans for surgery. Tell your anaesthetist, as the medicine slows stomach emptying.

Why Doctor Assessment Matters

A proper consultation checks your weight, height, blood pressure, blood sugar, kidney and liver function, medical and family history, and current medicines. Your doctor can also rule out other causes of weight gain, such as thyroid problems or PCOS. Medicine works best alongside a nutrition and activity plan, so expect lifestyle advice as part of treatment.

Eligibility Checklist

  • Do you have a BMI of 30 or more, or 27 or more with a weight-related condition?
  • Do you or a close relative have medullary thyroid carcinoma or MEN 2?
  • Have you had a serious allergic reaction to semaglutide?
  • Are you pregnant, breastfeeding, or planning pregnancy in the next few months?
  • Do you have pancreatitis, gallbladder disease, or diabetic eye disease?
  • Are you taking insulin or another diabetes medicine?
  • Do you have a history of depression or suicidal thoughts?

If you answered yes to any of the first four questions, do not start without speaking to a doctor first. For the others, your doctor will decide whether Wegovy is appropriate.

Wegovy Injection and Tablet

Wegovy is also available as a tablet in some countries, approved for weight management in adults with obesity or overweight and a weight-related condition. Whether a form is available in Malaysia, and its exact approved use, should be confirmed with the local product information and your doctor. Read more in our Saxenda, Wegovy and Ozempic guide.

Other Options to Discuss

If Wegovy is not suitable, your doctor may discuss other options, including structured lifestyle programmes, other weight-loss medicines such as Mounjaro, or treatment of a related condition. Compare the medicines in Mounjaro vs Ozempic vs Wegovy.

Frequently Asked Questions

Can I take Wegovy with a BMI of 27?

Only if you also have at least one weight-related condition, such as high blood pressure, type 2 diabetes or high cholesterol. Your doctor will confirm this.

PCOS alone does not rule you out, and weight loss may help some women. Read Wegovy and PCOS and discuss your full history with your doctor.

Can teenagers take Wegovy?
